What Is Suboxone?

A combination medication built to ease withdrawal and deter misuse.

Suboxone combines two medications: buprenorphine and naloxone.

Naloxone is an opioid antagonist, meaning it blocks the effects of opioids in the body — it's the same medication used clinically to reverse an overdose. Within Suboxone, the naloxone component is there to discourage misuse: it remains inactive when the medication is taken as prescribed (swallowed or dissolved under the tongue), but if the medication is injected instead, the naloxone activates and causes precipitated withdrawal — an unpleasant, immediate withdrawal reaction designed to deter abuse.

Two Dreams provides access to buprenorphine and probuphine through our admissions department, including induction, written prescriptions, and counseling with an on-staff psychiatrist. Outer Banks clients may start or continue Suboxone treatment as clinically indicated as part of their broader course of care.

An initial visit includes a review of medical and addiction history, urine drug testing, and office visits during the first month of treatment. Following the initial visit, patients are seen monthly for follow-up appointments.

"Two Dreams does not provide any medications." Prescriptions are written for the patient to fill at a pharmacy of their choosing.

How billing works for Suboxone treatment.

We do not accept insurance for the first month of treatment; instead, we work with each patient to draw up a payment plan if needed. From the second month of treatment onward, we investigate insurance coverage on the patient's behalf — with the exception of Medicare and Medicaid, which are not accepted for this service.

Common questions about Suboxone treatment.

What should I expect during my first appointment?

Your first visit focuses on education — understanding how buprenorphine works to prevent withdrawal and help you maintain daily functioning. Staff walk through the benefits and limitations of treatment, and you'll receive both written and verbal information on desired effects and potential side effects.

You'll review and sign treatment consent forms. The goal of treatment is explicit: to stop using. We treat patients age 18 and older, and we welcome pregnant women into care. With your permission, we'll also communicate with any private physicians you see, to help coordinate your care.

What is the difference between methadone and buprenorphine?

Both medications treat opioid dependence, but they work differently. Methadone is a "full" opioid agonist, whereas buprenorphine is a "partial" opioid agonist. Studies show buprenorphine tends to have better side-effect tolerance and causes milder long-term withdrawal symptoms; methadone may produce a more moderate to severe, prolonged withdrawal.

Is one medication better than the other?

Which medication is prescribed depends on the individual. Patients with mild-to-moderate dependence may prefer buprenorphine for its lower abuse potential, while patients with more severe dependence may benefit from the structure of a methadone clinic. History of injectable drug use, cost, and personal preference all factor into the decision.

Neither medication is objectively "better" — the right choice depends on medical history and a physician's recommendation.

How much does treatment cost? Do you accept insurance?

We do not accept insurance for the first month of treatment, though we will work with you to draw up a payment plan if necessary. Monthly follow-up treatment involves insurance verification (excluding Medicare and Medicaid), and patients fill their own prescriptions at a pharmacy of their choosing.
